Incubator Application

Candidates must complete this application in its entirety to be considered for acceptance to the Polk Institute Foundation Incubator Program.

Candidate Admission Criteria:

• A goal of $1 Million in revenue 3-5 years
• Willing/able to commit 10-12 hours per month to workshops and activites (Video Conference)
• Willing/able to commit 1 year to the Incubator Program

Additional Application Materials:

• You are required to supply a link to a 90 second video describing your company. This link can be private.
• You have the option of supplying a recent Pitch Deck.

Select applicants will be interviewed by a selection team and given an opportunity to discuss their businesses further. Acceptance and participation is afforded at the sole and absolute discretion of Polk Institute Foundation.
