Legacy Cohort Application

Candidates must complete this application in its entirety to be considered for acceptance to the Polk Institute Foundation Legacy Program.

 

Candidate Admission Criteria:

  • Minimum 10-years experience
  • Seeking growth and/or exit strategy
  • Willing/able to commit at least one year to the Legacy Program

 

Select applicants will be interviewed by a selection team and given an opportunity to discuss their businesses further. Acceptance and participation is afforded at the sole and absolute discretion of Polk Institute Foundation.
